Entry forms may be faxed to (707) 895-3011 only if no entry fees are required.
Remember to use a seperate entry form for each division.

These forms can be filled out on the computer and then printed. Remember to sign entry forms before you send them to the fair. You need Adobe Reader to view and print these files.
